How to participate in the QSCDK

Before you start

Give the information letter (PDF) and pamphlet (PDF) to the parents or guardians of your 5-year-old kindergarten students.

  • The letter includes a refusal form that parents can fill out if they do not want you to complete a questionnaire about their child. Collect the refusal forms signed by parents and return them using the prepaid envelope provided. If you receive a refusal form for a child after you have submitted their questionnaire, please send it to the ISQ anyway, as we will destroy the data for that child.

Familiarize yourself with the reference documents (this should take about an hour).

Have your list of 5-year-old kindergarten students at hand, which includes their permanent code, postal code and date of birth.

Review your students’ documents (e.g., report card, student record, kindergarten enrollment form) that may provide the following information:

  • Child’s first language(s)
  • Number of days absent
  • Repeated grade
  • Special needs
  • Childcare attendance in the year prior to starting kindergarten
  • Place of birth
  • Non-teaching professional services received at school

Make sure you have the username and temporary password you received by email or through your school principal.

Completing the questionnaires

Your username can be found in the envelope you will receive, and your password will be sent to you by email.

Ideally, you should complete all your questionnaires at once. The students do not have to be present when you complete the questionnaires.

Students covered by the survey

All 5-year-old kindergarten students are included in the QSCDK. You must therefore complete a questionnaire for each student in your class. If you have a mixed-level class (more than one grade level), only complete the questionnaire for students enrolled in 5-year-old kindergarten.

If you are two teachers for the same group, each teacher must complete the questionnaires for their assigned students.

You must have observed a child for at least one month before completing their questionnaire. Otherwise, please refer to question 15 in the Demographics section of the Early Development Instrument Guide (PDF).

How do I complete the questionnaires?

The questionnaires are stored on a secure web platform and are designed to be completed on an electronic tablet or computer. Do not use a phone. Make sure your web browser is up to date before you begin.<

  1. Ao to the questionnaires platform, then log in using your username and temporary password.
  2. Go to the questionnaires platform, then log in using your username and temporary password.
  3. Read the instructions.
  4. Click on the button for the questionnaires to be completed and verify that the “List of students” matches your group. If any students are missing from the list, add their names.
    • If a parent or guardian has filled out a refusal form for their child, let us know at question 0.
    • If the whole list is incorrect, contact the survey team at eqdem@stat.gouv.qc.ca or at 1-800-561-0213.
  5. Click on the pencil icon at the right of a student’s name to complete that student’s questionnaire.

After 30 minutes of inactivity, your session will automatically end. Don’t forget to save your answers regularly as well as before you exit a questionnaire, otherwise they will be lost.

Once you have submitted a questionnaire, you will no longer be able to edit it yourself. If you want to change any of your answers later, please contact the survey team at eqdem@stat.gouv.qc.ca or at 1-800-561-0213.

Security and confidentiality

You are the only person authorized to access the questionnaires for the students in your class. The information contained in the questionnaires is confidential and must be protected at all times.

Your username and password must be protected at all times and must not be shared with anyone. Complete the questionnaires in an appropriate location to ensure confidentiality.
