The Institut de la statistique du Québec is a government agency whose mission is to produce, analyze and disseminate official, objective and high-quality statistics on various aspects of Québec society.
To participate:
- Go to the questionnaire at statistique.quebec.ca/care
- Enter your identifier
- Create a password
- Complete the online questionnaire
If necessary, you can exit the questionnaire and return to finish it at a more convenient time.
If you have not submitted your online questionnaire, the ISQ will call you to obtain your answers by telephone. If you are not available when we call, just let us know what time would be more convenient for you.

The questions will ask about your health status, your experiences accessing health and social services, and your use of certain services. Please provide the most accurate answers possible. If you are unable to provide an accurate answer to a given question, please answer it to the best of your ability, as this information will still be invaluable to us. If you prefer not to answer a question, you can skip it, regardless of your reason for not answering.
