The objective of this survey is to collect data on the health of the Québec population in order to tailor health and well-being programs to the specific characteristics of the different regions in Québec.
The Institut de la statistique du Québec is the public agency responsible for providing reliable, objective and relevant statistics about Québec society.
To participate:
- Go to the questionnaire at statistique.quebec.ca/health.
- Enter your identifier.
- Create a password.
- Complete the online questionnaire.
The Institut de la statistique du Québec will follow up by phone with those who have not submitted their online questionnaire. If you are not available when we call, just let us know what time would be more convenient for you.

We will respond to your request within 24 hours during our office hours, Monday to Friday between 8:30 a.m. and 4:30 p.m.
We will ask you to provide information about your health, your lifestyle habits, and your well-being. There will also be questions about your living and work environments, housing, physical activity, and certain conditions that can impact health. Lastly, a few questions will serve to describe the different realities of the population for statistical purposes.
Your name was selected at random, just like that of several thousand other people in Québec, so that you can participate in the Québec Population Health Survey.
This survey is conducted in accordance with the Act respecting the Institut de la statistique du Québec, and your contact information was obtained under this Act.
Your participation or that of your child in this survey is voluntary but important, because it will help the Ministère de la Santé et des Services sociaux (MSSS) to collect data about the health and well-being of the population in your region in order to adjust the programs and services offered. Your participation is essential to learn about the health of people in your region so that the services offered can be better tailored to their needs.
For quality and training purposes, telephone conversations with Institut de la statistique du Québec agents are recorded. If you do not want your conversation to be recorded, simply notify the agent at the beginning of the call.
The Act respecting the Institut de la statistique du Québec guarantees that your answers will be kept confidential and will only be used for statistical purposes.
All those working at the Institut de la statistique du Québec have been sworn in before a commissioner and are bound to maintain secrecy and confidentiality.
The Institut de la statistique du Québec has entered into an agreement with the MSSS, to whom the data collected will be sent, except for your name, address, and phone number. Your answers will be kept confidential and will only be used for statistical purposes.
Yes. The ethics committee appointed by the Institut de la statistique du Québec reviewed the survey to ensure it is conducted in a way that respects participants and protects their dignity.
The questionnaire is personal and can only be completed by the person who was selected for the survey. The person selected represents people in their age group and region.
