The objective of the survey is to better understand how people are moving, their perception of an active lifestyle, and their use of recreational and sports facilities. The input of all individuals is appreciated, whether or not they are physically active.
The Institut de la statistique du Québec (ISQ) is the public agency responsible for providing reliable, objective and relevant statistics about Québec society.
Follow these steps:
- Go to the questionnaire at statistique.quebec.ca/e336en.
- Enter your identifier.
- Create a password.
- Complete the online questionnaire.
If necessary, you can exit the questionnaire and return to finish it at a more convenient time.
If you have not submitted your online questionnaire, the Institut de la statistique du Québec will call you to obtain your answers by telephone. If you are not available when we call, you can let us know what time would be more convenient for you.
Make sure you entered your identifier correctly, without any spaces before or after.

We will respond to your request within 24 hours during our office hours: Monday to Friday between 8:30 a.m. and 4:30 p.m.
We will ask you questions about which physical activities you did in the past few seasons and where you did them. We will also ask about your opinion and perception of an active lifestyle, then we will continue with a few questions about your health and certain characteristics for statistical purposes only. Only you can complete your questionnaire.
Your name or your child’s name was selected at random, just like that of several thousand other people, so that you can participate in the Québec Survey on Health, Physical Activity and Active Lifestyles 2026-2027.
This survey is conducted in accordance with the Act respecting the Institut de la statistique du Québec, and your contact information was obtained under this Act.
Your participation or that of your child is voluntary, but important. Your answers will help improve the physical activity and sports services offered to the population.
For quality and training purposes, telephone conversations with ISQ agents are recorded. If you do not want your conversation to be recorded, simply notify the agent at the beginning of the call.
The Act respecting the Institut de la statistique du Québec guarantees that your answers will be kept confidential and will only be used for statistical purposes.
All ISQ employees have been sworn in before a commissioner and are bound to maintain secrecy and confidentiality.
The ISQ has entered into an agreement with the Ministère de l’Éducation (MEQ) and the Ministère de la Santé et des Services sociaux (MSSS), to whom the data collected will be sent, except for your name, address and phone number.
Yes. The ethics committee appointed by the ISQ reviewed this survey to ensure it is conducted in a way that respects participants and protects their dignity.
The questionnaire is personal and can only be completed by the person who was selected for the survey.
