You were randomly selected to participate in the Québec Population Health Survey 2020-2021 just like thousands of other Quebecers.
We only obtained the personal information needed for the survey, including your contact information, from the Régie de l’assurance maladie du Québec (RAMQ) under an agreement approved by the Commission d’accès à l’information in accordance with the Act respecting Access to documents held by public bodies and the Protection of personal information.
Your participation in the survey is voluntary, but important since it will allow the ministère de la Santé et des Services sociaux (MSSS) to better meet the needs of the population in terms of health protection. In addition, your participation is essential for producing a specific portrait of the health status of the population in your region so programs and services can be designed or improved to better respond to local needs.
For quality and training purposes, phone calls with Statistique Québec agents are recorded. However, if you don't want the call to be recorded, simply notify the agent at the beginning of the call.
The Act respecting the Institut de la statistique du Québec guarantees that the information you provide will be kept confidential and will only be used for statistical purposes. All Statistique Québec employees have taken an oath before a commissioner and are bound to maintain the secrecy and confidentiality of the data they collect.
Under the terms of the agreement between the ministère de la Santé et des Services sociaux and the infocentre de santé publique du Québec, to whom we will transmit the data collected, with the exception of your last name, first name, address, postal code and telephone number.
All of your information will be kept confidential and will only be used for statistical purposes.
In accordance with the Act respecting Access to documents held by public bodies and the Protection of personal information, the Commission d’accès à l’information du Québec has informed the Institut de la statistique du Québec that an information letter must be sent to the parents or guardians of any minor participating in our surveys. However, if you have just reached the age of majority, your parents may also have received a letter. This sometimes happens because ages are calculated based on participants’ year and month of birth only.
The questionnaire is personal and can only be filled out by the person who was selected for the survey.
